Fillable POS Form

This Program of Study must be prepared in consultation with the doctoral advisor. It must reflect the degree requirements listed in the Graduate Catalog. Any changes to the degree requirements as shown on the POS must have written approval and justification of the graduate advisor and the Program Director prior to submission to Graduate College. Information given must be complete.

 

Course Substitution

Petition - Course Substitution.pdf 

This form is to substitute courses in your degree plan as approved by your academic advisor. This form also requires a written rationale for each substitution. Your advisor or dissertation chair can help you with this.

 

Drop by Instructor

Drop by Instructor Form.pdf 

This form used by instructors to drop students from courses after the official UTRGV "W" date.

 

Request for Grade of Incomplete

UTRGV - Incomplete Grade Form.pdf 

This form is for students to request an "I" for incomplete. This requires the signature of your instructor and must meet the requirements for an Incomplete as per the Graduate College Handbook. 

 

Reduce Attendance

Petition - Change POS_Reduce Attendance_Stop-out.pdf 

This form is for you to petition for a stop-out, change the timeline of your POS, or reduce the number of enrolled hours below 6 hours a semester. This form requires the approval of your advisor or Dissertation Chair and the C & I Program Coordinator.There are specific requirements for each of these that must be met as well.

 

ACE Forms:
 

ACE Test Request

Advancement to Candidacy Examination _ACE_.pdf

This is the Advanced to Candidacy Examination (ACE) application form. It needs to be completed by October 1st for Fall, Feb. 1 for Spring, and June 1 for Summer. Include the Application for Dissertation Committee Form.

 

Dissertation Forms:

 

Application for Dissertation Committee

This is the application for the dissertation committee. This committee formed by doctoral faculty will help you and guide you through the design and execution of your doctoral-level research project. Make sure to submit this with your ACE application.

 

Certification of Completion of  Dissertation

This form is signed by faculty at your dissertation defense and then submitted to the Graduate College.Or if you need to make corrections, the form is submitted after these corrections are complete. It certifies that your dissertation has been approved, and can be published in ProQuest so you can graduate.

 

Change of Committee Form

This form is to request a change of one or more of the Dissertation Committee members. Use this form also, if you are going to request a committee member who does not work at UTRGV. There are some additional requirements for external committee members.

 

Dissertation   Manual

Dissertation Progress Report
